Why paper logs fail inspection
Rarely because the care was poor. Usually because the record was written hours later, in summary, by someone reconstructing a shift from memory — so it is thin where it needs to be specific, and it cannot be searched when somebody asks what happened on a particular date. Recording on shift solves the first problem; recording into a structured form solves the second.
Forms you can change yourself
Your recording practice is not the same as another provider's, and it changes after an inspection, a serious incident or a new placement. The form builder is drag-and-drop so you can change a form without raising a support ticket and waiting a fortnight for it.
Drafts are not records
A half-finished log is tracked as a draft, separately from what has been submitted. That sounds like housekeeping; in practice it is what stops a shift's recording quietly disappearing because somebody was interrupted and never came back to it.
Where the line is. A form is only as good as what is typed into it. CareOptix does not check the quality of a log, prompt for detail, or judge whether recording practice meets the standard — a manager still reads them.
