Where incident recording goes wrong
Two places. The form is written somewhere that is not the record — a paper pad, an email to a manager — so it takes a search to find it later. And the concern it raises sits in that email until someone reads it. Structured capture fixes the first; a flag on the dashboard fixes the second.
The forms are yours
Providers record incidents differently, and change how they record them after an inspection or a serious event. The form builder means that change takes an afternoon rather than a change request.

An incident form belongs to the young person's timeline as well as to the pile of forms submitted that month. Both views matter: one is how you understand a placement, the other is how you spot a pattern across a home.
