What is an improvement plan?
An improvement plan is the set of actions arising from a review, an inspection or a visit, each with an owner, a date and evidence of completion.

</figure>It is what turns a finding into a change. A review that identifies problems and produces no plan has documented a home rather than improved it, and that distinction is exactly what an inspector looks for.
Actions from different sources — inspection, independent visits, the quality of care review, incidents — belong in one place, because otherwise nobody can say what is outstanding for the home overall.
Evidence of completion matters more than the tick. A photograph, a training record or a revised document attached to the original finding is what shows what was actually done.
What goes wrong in practice
- Actions with no named owner, which reliably means nobody does them.
- Separate plans for inspection actions and review actions, with no combined view.
- Closed on a spreadsheet with nothing behind the tick.
